Some are biochemical.
An excellent discussion of biochemical diseases is given in “Inborn Metabolic Disease” Saudubray et. al. eds. and “The Metabolic and Molecular Bases of Inherited Disease” [MMBID] Scriver et. al. eds.
One biochemical cause of schizophrenia is methylene tetrahydrofolate reductase deficiency.
The authors of MMBID make the essential point.

“There is considerable evidence that many inherited metabolic disorders remain undiagnosed or misdiagnosed. There are a number of reasons the most important of which is the fact that many physicians are not aware of most of the highly specific symptoms or syndromes that are excellent keys to the diagnosis and consequently do not perform comprehensive examinations.”

Anyone who treats some one with severe mental illness without performing a full biochemical medical examination is guilty of medical malpractice. It is tragic to miss a treatable biochemical illness.
